Close the Loop: A Safety Finding Needs a Verified Fix

Key message: Convert reported hazards into assigned, effective, and communicated corrections.
Know the hazard
Team: General Small Business. Topic: Safety Leadership.
Finding a hazard is only the beginning. An inspection form can be complete while the unsafe condition remains. A useful corrective action has a clear problem, an appropriate interim control, an owner with authority to act, and a way to verify the result.
Use the controls
Prioritize serious exposure and keep people protected while a permanent solution is arranged. Describe the action precisely enough that another person can tell whether it happened. 'Address storage' is vague; identifying a safe storage arrangement, implementing it, and checking actual use creates a reviewable outcome.
Work within the safe plan
After the correction, observe the work and ask employees whether the control is practical. Look for new problems introduced by the change. A closed work order or purchase receipt does not automatically prove exposure was reduced. Share the result with the person who reported the concern and with other affected workers. If a finding repeats, investigate why the previous correction failed. Today, we will take one open item through the whole cycle and agree on what evidence will establish that it is ready to close.
Discuss with the crew
A blocked exit finding is marked closed after boxes are moved. The same delivery process blocks it again the following week.
Question: Why did the correction fail?
Answer guidance: It removed one instance without changing the receiving and storage conditions that caused it.
Question: What would stronger verification include?
Answer guidance: A workable receiving arrangement, worker understanding, and checks during actual deliveries showing the exit stays clear.
Prepare, act, and follow up
Leader preparation: Bring one open inspection finding and a de-identified example of a completed correction.
Put it into practice: Assign one real finding an owner, interim protection, completion date, and verification method.
Supervisor follow-up: Review completion and effectiveness with the reporting worker and reopen the item if the control fails.
