A slab opening can move from one crew's responsibility to another without anyone noticing the protection disappeared. Cutting, coring, form stripping, and utility installation all change access. Do not step onto a loose sheet or assume a covered area is designed to carry people and materials. The protection must suit the opening, work, and expected loading and stay in place while the hazard exists.
Use the controls
Use the project's approved covers, guardrails, or other required protective system. Covers must be secured and identified as the applicable requirements demand; loose scrap is not a planned system. Do not remove protection without the authorized arrangement for the resulting exposure. If a task needs access through the opening, coordinate the work so people above, below, and beside it remain protected. Tools and concrete debris can fall through as well as workers.
Work within the safe plan
At handover, account for every opening affected by the job, including small cores and spaces exposed when forms are removed. Report missing, damaged, or displaced protection immediately and prevent access until corrected. Do not carry a load that blocks the view of the walking surface. Recheck after another trade works there, after material deliveries, and before lighting is reduced. A clear opening register helps the project keep protection continuous rather than assuming that whoever made the hole will always be nearby to explain it.
Discuss with the crew
A core-drilling crew leaves a loose board over the hole for the next trade.
Question: What is missing?
Answer guidance: An approved, suitable, secured and identified protective arrangement and a clear handover.
Question: Does opening protection only address people on the upper level?
Answer guidance: No. Falling tools, debris, and material can expose people below as well.
Prepare, act, and follow up
Leader preparation: Bring the opening register and cover-control instructions.
Put it into practice: Identify each opening on the register and assign the handover responsibility.
Supervisor follow-up: Inspect opening protection after drilling, stripping, and trade changes.
